I declare that I am aware that the activities of mountaineering in all its forms, in which the equipment in question is used, are inherently dangerous and that the improper use of the equipment as well as the lack of functionality of the equipment can lead to serious injuries or even death.

I also expressly declare, for the purposes and effectiveness of Art. 1341 and 1342 of the Civil Code, that I have taken note of and accept Annex A ("General Conditions") to this Rental Form. He/she further declares that he/she is aware of and expressly accepts in particular the provisions in Art. 4 ("Increase of the fee in case of late return"), Art. 5 ("The equipment may not be used by third parties"), Art. 6 ("Consideration of the modalities of use, restriction of the purpose of use, awareness of danger") and Art. 7 ("Dangers of mountaineering, liability of the borrower and exclusion of liability of the lender"), to which he/she has been made aware.
